Terms and Conditions

(Applies to RxDirectCare and MDDPC)

RxDirectCare and MDDPC are programs offered by MDHIN Corporation. MDHIN Corporation facilitates access to these membership-based healthcare services through a network of independent providers and pharmacies. All programs operate under the terms described below.

1. Membership-Based Services
   RxDirectCare and MDDPC provide access to healthcare services through a membership-based program. Membership is NOT health insurance, does not replace insurance, and does not meet ACA minimum essential coverage requirements.
2. Covered Services
   Membership includes access to provider or pharmacy-based consultations, care coordination, and related services as outlined in your plan tier. Visit counts, point-of-care testing (POCT), and other benefits are limited to the caps specified in your membership plan.
3. Medication Preference and Provider Discretion
   During certain assessments (including weight loss programs), you may be asked for a medication preference (e.g., semaglutide, tirzepatide, or no preference). This preference is for informational purposes only and does not guarantee a prescription. Final selection, dosage, and treatment plan will be determined solely by a licensed healthcare provider.
4. Eligibility and Compliance
   You must provide accurate and complete health information. Misrepresentation of medical history may result in denial or termination of services without refund. Providers may refuse services if you do not meet clinical eligibility criteria.
5. Cancellations and Refunds
   Memberships renew automatically each month unless cancelled in accordance with your plan’s cancellation policy. Cancellations must be requested via email to subscriptions@mdhin.com. By enrolling, you agree to a 90-day cancellation policy. Charges will continue for 90 days after the end of the billing term in which the cancellation request is received. Unused visits do not roll over to future months. Certain services (e.g., lab work, specialty testing) may be non-refundable once performed.
6. Telehealth and Privacy
   Services may be provided in person or via telehealth. By participating, you consent to telehealth encounters in compliance with applicable state laws. All health information is protected under HIPAA. Please review our Privacy Policy for details on data use and storage.
7. Limitations of Liability
   RxDirectCare and MDDPC are not responsible for adverse outcomes related to services provided by independent providers or pharmacies in our network. Membership fees do not cover costs of services or medications obtained outside of the program.
8. Provider and Pharmacy Network
   We partner with independent licensed providers and pharmacies. Inclusion in the network does not imply employment or agency. Providers retain independent clinical judgment.
9. Lab Testing & Specialty Services Disclaimer
   From time to time, you may be offered optional laboratory or specialty testing services (including but not limited to pharmacogenomic [PGx] testing and genetic testing). These tests are voluntary and not required for membership. Results may be shared with your provider for informational purposes only. RxDirectCare and MDDPC do not provide medical advice and do not interpret test results beyond facilitating access to the testing service. All medical interpretation, diagnosis, or treatment recommendations are made solely by your licensed healthcare provider.
10. Changes to Terms
    We reserve the right to update these Terms & Conditions at any time. Continued participation in the program constitutes acceptance of the updated terms.
11. No Guarantee of Results & Limitation of Liability
    RxDirectCare and MDDPC make no guarantees regarding the outcomes, results, or effectiveness of any products, services, medications, or treatment plans obtained through the program. Participation in any program is voluntary and at your own risk. All clinical decisions are made solely by independent licensed healthcare providers. You should always consult with your own primary care provider or other qualified healthcare professional regarding any medical condition, diagnosis, or treatment plan, including before starting or changing any medication. To the maximum extent permitted by law, RxDirectCare, MDDPC, and their affiliates shall not be liable for any injury, adverse effect, or other damages resulting from participation in the program or reliance on any services provided.
12. Privacy Policy
    RxDirectCare and MDDPC are committed to protecting your privacy. We collect, use, and store personal health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. Your information will only be shared with authorized healthcare providers and third parties as required to deliver services, process payments, or comply with legal obligations. We implement administrative, technical, and physical safeguards to protect your data. By participating in our programs, you consent to our collection and use of your information as described.
13. Provider Availability
    While we strive to ensure access to providers within our network, we do not guarantee the availability of any specific provider at any given time. Appointment availability may vary based on provider schedules, state licensing restrictions, and patient demand. In some cases, you may be referred to an alternative provider or asked to reschedule. RxDirectCare and MDDPC are not liable for delays in care due to provider unavailability.

Weight Loss Program Addendum

1. EligibilityParticipation in the weight loss program, including GLP-1 therapies, is subject to review of your medical history, applicable clinical guidelines, and final approval by a licensed provider.
2. Medication Preference If eligible, you may indicate a preference for semaglutide, tirzepatide, or no preference. This is for informational purposes only and does not guarantee a prescription.
3. Provider Discretion Final treatment decisions, including choice of medication, dosage, and treatment plan, are made exclusively by the prescribing provider.
4. Risks and Responsibilities All medications carry risks and potential side effects. You should discuss all medical conditions, allergies, and current medications with your provider. You are responsible for following provider instructions and reporting any adverse reactions.
5. No Medical Advice from RxDirectCare or MDDPC RxDirectCare and MDDPC only provide access to products and services and do not provide medical advice. All medical decision-making and prescribing authority rests with your licensed healthcare provider.
6. No guarantee is made regarding the success, safety, or effectiveness of any weight loss treatment. Participation is voluntary and at your own risk. By participating in these programs, you acknowledge that RxDirectCare and MDDPC are operated by MDHIN Corporation and that all services are subject to these Terms & Conditions.
